1. Instead of you handling image , you send the image to my given api. www.abc.com/upload
2. I store it , and return you the file name. eg : diddy.jpg. If nsfw I tell you in response.
3. You store the image name.
4. In your site you get the image using eg : www.abc.com/image/diddy.jpg

There is something different about building own stack and building over it.
Think all the base tools made here (all kinda shitty but all indie and not corpo ) and you use tools to build your own stuff over it. It kinda have a hacker feel. Something like you can build your own react even when there is already one available. But it has it different charm and then people using it adds cherry on the top.
